     ===================== URINARY CLINICAL PATHOLOGY ======================

         Increased alkalinity may result following a full meal, after
         consuming large amounts of citrus fruits, or as a result of
         taking alkalies.

         Urine may be alkaline in chronic cystitis and retention of urine
         in the bladder due to decomposition by bacteria.

         Long standing urine samples result in bacterial decomposition of
         urea and the liberation of ammonia causing an alkaline reaction.

         Glucose is normally present in 0.01 to 0.03 gram per 100 cc.
         of urine.

         In diabetes mellitus sugar usually appears in the urine when the
         level of glucose in the blood rises above 160 mg. per 100 cc.
         (varies from 114 to 216 mg.).

         In renal diabetes glucose appears in the urine with normal or
         low blood levels.

         Glycosuria may be present after brain injury and after coronary
         thrombosis.

           * Consider ordering a serum glucose.

         Urobilinogen is formed in the intestine by the action of
         bacteria on bile pigment.
         A temporary increase may be caused by constipation.

         Consider hemolytic jaundice.

         Small round epithelial cells are abundant in glomerulonephritis,
         nephrosis, hemolytic anemias and hemochromatosis.

         Epithelial caudate and cylindrical cells are derived from the
         transitional epithelium which lines the bladder, ureters, and
         pelves of the kidneys.

         Indole is the product of putrefaction of protein in the
         intestinal tract and is excreted as indican. It is increased in:

           1) Diets rich in proteins.
           2) Intestinal indigestion and intestinal obstruction.
           3) Diseases of the stomach associated with decreased hydro-
              chloric acid, gastritis, and in gastric neoplastic disease.
           4) Diminished bile flow.
           5) Decomposition of protein in other parts of the body as
              occurs in peritonitis, empyema and large abscesses.

           * Order a psyllium seed bulking agent bowel cleansing program.

           * Order CEA if clinical signs are suggestive of bowel malignancy.

     ====================== METABOLIC URINE ANALYSIS =======================

         Suspect potassium deficiency.  Consider ordering a serum
         poatassium and electrocardiographic examination.
